Home Modifications Assessments

Ensuring a safe and accessible home environment is crucial for older Australians who wish to remain at home. At Key Elements Health we conduct home modifications assessments to help you maintain your independence while living comfortably in your own home.

During a home visit, we assess your living environment to identify any areas that may benefit from modifications. These assessments aim to enhance safety and accessibility, enabling you to navigate your home safely and with ease. Based on the assessment, we may recommend various modifications such as:

  • Installation of handrails and grab bars,

  • Widening doorways for easier access,

  • Adding ramps for wheelchair accessibility,

  • Bathroom modifications to ensure safety during personal care activities.

Equipment and Assistive Technology Assessments

Choosing the right equipment and assistive technology is essential for maintaining mobility and comfort at home. Key Elements Health conducts in-home equipment and assistive technology assessments tailored to older adults to ensure you, or your family have the right tools to support your daily activities.

We provide clinical assessments, expert advice, and product recommendations tailored to your specific needs. This includes seating and positioning systems, mobility aids, and other assistive technologies. We consider various factors during the assessment, such as:

  • Mobility and posture needs,

  • Safety and injury prevention,

  • Continence care and overall comfort.

Our goal is to ensure that the equipment and assistive technologies prescribed are not only effective but also enhance your quality of life. We provide educational support to help you and your caregivers understand how to use the equipment properly, ensuring that it meets your needs in both aged care settings and home care environments.

Aged Care FAQs

  • When trying to understand occupational therapy, people tend to get stuck on the word occupation, relating it purely to a job. Occupational therapist use the word "occupation to mean the things we do everyday, how we occupy our time. Therefore Occupational therapy is therapy for things we do everyday.

    Occupations include looking after ourselves (self cares or pADLs) such as eating, dressing, bathing, toileting etc; looking after/organising our environment (self management or iADLs) such as washing, cooking, cleaning, shopping, engaging in meaningful activity.

    Occupational therapists work with people to find ways to complete these tasks that may have become difficult after a change.

    1. Referral - This usually involves receiving a referral from a support coordinator or other allied health professional

    2. Discussion - Phone call or face to face interview to determine if the services Key Elements health provides are inline with client’s needs

    3. Service agreement - If both parties agree that Key Elements Health is the right fit, then a service agreement is provided and signed.

    4. Assessment - Complete a functional capacity assessment (if required). Determine needs and goals

    5. Intervention - Get into some therapy!

  • Our Occupational Therapists can assess, recommend and support a variety of modifications to make your home safer and easier to navigate. This might include installing handrails and grab bars, widening doorways for better access, adding ramps for wheelchairs, or adjusting your bathroom for safer personal care. Each recommendation is tailored to your needs, with the goal of helping you stay independent in your own home.

  • If you’re finding it harder to move around, complete daily tasks or stay comfortable and safe at home, you might benefit from specialised equipment or assistive technology. Our Occupational Therapists will visit your home to assess your situation and suggest the right equipment for you, whether it’s a wheelchair, walker, or a specific seating and positioning system. We ensure that whatever is recommended is clinically suitable and helps you live as independently as possible.

  • Absolutely. Our Aged Care Occupational Therapy services are available through your Home Care Package. We work with your Home Care Package manager or provider to make sure the supports we provide fit within your care plan and budget, ensuring you get the most from your funding to support your independence and wellbeing at home.
